Transition Plans can follow different formats and there is not one single way to do them. Ideally Personal Futures Planning should be at the start of the process to bring the team together to focus on the individuals' preferences and dreams. Once a general vision of the desired future becomes a bit clearer, a Transition Plan can help to put that into place through specific action steps.
